Personal Presence of the Birth Certificate Holder (if adult) or one of the parents (in case of a Minor) is necessary.

Valid Passport original and a photocopy (First, Last, Address and Valid Visa Page ) of the Birth Certificate holder or one of the parents ( if a Minor)

Birth Certificate Original attested by Notary Public of the District where the child was born and Home Department of the Concerned State/Union Territory where the child was Born and a photo copy

If the Document was issued by authorities in UAE, The document has to be attested by the Ministry of Foreign Affairs, Dubai/Sharjah.
